Insurance Accepted

We accept all major insurance plans including local HMOs, Fidelis and Medicaid. New patients are always welcome.

Newborn checklist

1. Add your newborn to the insurance policy immediately.
2. Name any one doctor as Primary Care Physician when required. Listing one will allow coverage for all the doctors. (do not list the name “Summit Pediatrics”) (see list of doctors to the left)
3. Select one office address
4. Effective date: Date of Birth

New and Established Patients

For new and established patients with an HMO, Fidelis or Medicaid, follow the instructions above. Be sure that the effective date is on or before the date of service.

Insurance Cards and Co-Pays

YOUR INSURANCE CARD MUST BE PRESENTED AND COPAYS ARE DUE ON THE DATE OF SERVICE.

If you have more that one insurance, all cards must be on file in our office.
